August 26, 2010; Source: NPR | This article describes Martin’s Point, a nonprofit medical practice that is implementing the “medical homes” concept. As it is described here, Martin’s Point has doctors, medical assistants, and nurses working in teams to ensure that patients are seen without a wait partly by ensuring that all professionals are functioning “at the top of their license”. The team profiled here also makes active use of the cumulative information they can draw from electronic medical records to judge their own effectiveness in helping patients to control chronic conditions.
